Apply and get verified
Tell us about your agency. We verify your state license, CCN (if Medicare-certified), and NPI before you receive any leads.
Receive matched requests
When a family in your service area submits a request that fits your services, you receive it by email with their contact preferences.
Use your free credits
Each delivered lead uses one credit. Out-of-area, duplicate, or unreachable leads can be disputed and refunded. We’ll let you know before your free credits run out.
Simple, flat pricing
After your free leads, you’ll be able to choose a flat plan with prices published in advance. Pricing will never be based on admissions, patient outcomes, or a share of your revenue.

Our standards
- Families opt in with a clear, unchecked consent box naming up to 3 agencies.
- Requests collect minimal health information.
- We don’t resell leads, and agencies may not resell them either.
- Agencies are responsible for their own calling, texting, and privacy compliance.
- Any paid placement will be clearly disclosed to families.
